Clinic Paperwork

It is imperative that you fill out paperwork ahead of time or get to the office 1/2 hour early for your first visit as paperwork can be time consuming. Arriving at the time of your appointment and not having complete paperwork may result in having to reschedule your visit to another day.
We make every attempt to confirm your insurance benefits, but please understand it is your responsibility to know if you need referrals, or have certain benefits included with your plan. The sooner we have your information the better able we are to help you avoid any surprises.
